  • While it is possible there is an issue in a Patient's Evaluated History and the resulting forecaster recommendations, often the issue is related to documentation error, i.e. a mistake was made when entering a vaccine.

    The best practice is to match the CVX code in the graph to the CVX code displayed after the vaccine name in the evaluated history:

  • Does the CVX code displayed in the Registry match the vaccine in the code guidance sheet?*

  • The logic built into the backend of the evaluated history and forecaster is complex. While a vaccine may fall outside of the standard schedule, there are "absolute minimum/maximum" limits that will still result in a "Valid" evaluation.

    Please review the vaccine in question to make sure what you are seeing does not fall into a "Special Situation" scenario.

  • The Immunization Registry team cannot view or make changes to a specific practices Electronic Health Record (EHR), so the first step to evaluating an issue with automatic (HL7) messaging is to have the practice reach out to their EHR Vendor (such as Epic).*
